If you want to instruct us, either complete the appropriate forms and send us all the relevant papers, (our address is at the bottom of this page) or contact David Inch.

All the forms are in PDF format. You can complete them online or download them to fill in later. Once you've filled them in, just sign them and return them to us.

You will need PDF software such as Adobe Acrobat to access and complete these forms. If you do not have this already you can download it, free of charge, from the Adobe website. To save the forms to fill in later, right click (CTRL-click on Mac) and select "Save Link As..." or "Save Target As...".
